2.  Fill out each page as completely as you can (please write clearly).

 

3.  Fax all pages to (541) 535-1417 or bring them with your to your appointment.

Although you may fill out the forms in our lobby, we believe this added convenience allows for a better overall experience, saves you, our patient, valuable time and offers heightened privacy.
