1.  Choose the correct paperwork below:

2.  Print out the packet and fill out each page as completely as you can (please write clearly).

 

3.  Fax all pages to (541) 535-1417 or bring them with you to your appointment.

Although you may fill out the forms in our lobby, we believe this added convenience allows for a better overall experience, saves you, our patient, valuable time and offers heightened privacy.
